With my current and very limited knowledge, I am not concluding that fatigued adrenals do not occur. I believe it is very possible.

Imagine if the hypothalamus is secreting loads of CRH and in response the pituitary gland is secreting loads of ACTH to cause the adrenal glands to secrete more glucocorticoids, because the hypothalamus is recognizing that the current level of glucocorticoids in the body isn't enough to respond to the threat being asserted at the present moment.

Then imagine that while this is occurring that all parasympathetic nervous system processes (digestion for one) are being halted in response to the threat being asserted.

Now imagine that this onslaught occurs regularly for several years (I realize this is extreme, but possible).

You don't think that it is possible for those organs to 1) give out entirely or 2) produce at a sub-optimal level because they have been damaged from years of over-production ?

I think if you believe that the adrenals are only being told what to do, you also have to accept that when told to do something for so long, when they are not designed to perform at such a high rate at a continuous pace, there is the possibility for them to greatly weaken and become under producers.
