Well that's the extreme form of starvation you see in the media of people who've been calorie-restricted for a very long time in really bad conditions, but I agree that's what I first think of too when I hear that word and it's why usually try and use the phrase prolonged under-eating/calorie-restriction instead to not confuse people that think you have to skin & bones and at the brink of death to be classified at suffering from starvation.

You can be mentally anorexic while still obese. You just won't be physically anorexic until your body uses up all your stored body fat and starts eating at your muscle to get fuel from starving yourself.

The average men's BMR is 1500 and the average women's is 1200. That's why a lot of starvation diets restrict at those numbers and why no one can last eating that little longterm unless, as you say, they lie in bed all day. So many people in the AF section ate those amounts or even fewer while going to school fulltime, working fulltime, and exercising multiple days a week and then they wonder why they developed all those symptoms they think is "AF".
