One thing that leapt out at me was the "sign" of spilling a glass of water. Water almost always equates to spirituality &/or emotions, and in your case, with the medical career plans and all, could *also* symbolize your health.

Do you feel you are "spilling" your spiritual health or your emotional health? If so, it may be time to sit back and review where you really want to be. Your goal to be a doctor and brign true health to the world is extremely noble and very admirable, however, perhaps it's not *really* what your emotional and spiritual self needs right now? Could you possibly need to "spill" your true gifts out onto others in some fashion other than medicine right now?

Just some thoughts that popped out at me when I read your post.
