Hi! Thank you for post. I am quite sure I have the exact same (or nearly exact same) problem!
I just recently have been exploring gut as the problem and am quite certain it is. You seem younger perhaps, and my condition may have been longer or more gradual. I mention this as the triggers for rash, and malaise are quite clear now though. For example, coffee almost instantly causes a rash, and eating certain things gives me tingling in the areas the rash will appear, almost imperceptibly, but within minutes of eating.
I really have been searching for a long time as this has affected me perhaps 7 years and gotten worse slowly and in waves.
I decided to post the things that I have found and recently worked (affected) the condition!
Here are the two things that seem to affect the condition (I'm not a doctor so please be careful and consult a doctor too first, especially with the first medicine as it is dangerous I believe):
1) Oral Antifungal medication (Flucazole and or Diflucan) This without a doubt affect the rash and caused it to go away within 3 days and the effects felt within a day. The rash cleared for a month at least. This medicine made me nauseous and may have actually lead to bruising on my arms! Really pretty intense stuff and definitely worth questioning, but it without a doubt had the desired affect and led me to believe it is indeed at least partially a fungal infection/imbalance. Perhaps I did not stay on it long enough or balance my body after to achieve long term success.
2) I've been on a Chinese Medicine (as I reside in Shanghai) named 百癣夏塔热片 or written as Baixuan Xiatare Pian. It seems to essentially be herbal and a laxative as it really just has me in the bathroom at least 3 times a day - and to be honest I have never felt better because of it! Really it seems to be having a long term impact and definitely is making me remember what it feels like to be healthy again. I am taking 3 pills 3 times a day. and will for about a month to two months I believe.
Those two things said it is still in process. I'll let you know.
As a side note I believe this condition came about in me after living in Korea for 2 years and not eating so well there as well as being put on an IV of
Antibiotics for Bronchitus. I always wonder if that upset the balance of my body and the more I read about digestive disorders the more I think it could be the case. - just thoughts though no solid scientific analysis.
I'd love to know if you solved this as well and would be happy to post pictures here too.
