Everything you have stated over the course of the past couple of weeks rings of some form of aspergillosis, IMO.

If it were me at that stage (with all this refusal and ignorance on the part of my MD), I'd start by ordering this kit:

It includes a petri dish containing a medium, and is obviously used to detect mold in the environment. HOWEVER, customer can send the sample to the manufacturer mail in lab for identification.

Cough or spit into it (anything containing saliva, anything that might come up from lungs, etc.) and allow 48 hours to pass. If you have mold in your respiratory tract, mouth, throat, etc., you WILL see it growing within that period. Allow at least three days for growth. If you see growth, send it to the lab for analysis.

Cheap, simple, and straight forward.
