No wonder you are having such a rough time, you're starving yourself!!! And your ND should get their licence revoked for recommending a starvation diet for you.

Since high protein/fat diets are screwing everyone up (as it did with me), you need to either eat starches (grains or potatoes, etc) or sweet fruits as your staple. Both are high in calories and low in fat. I usually eat mostly fruit all day until the evening when I eat cooked starches. Some days I'll eat very high fruit, then a big salad in the evening. Days I run out of fruit I'll just eat cooked starches all day as my staple; rice, pasta, potatoes, oatmeal, and also high starch veggies like carrots and such. Include plenty of leafy greens daily.

If you think you have leaky gut, then only eat fruit and tubers (potatoes, sweet potatoes, yams, etc) as your stable until your intestines heal.

The sweeter fruits are best because they are higher calorie. I feel best when I eat them all day, but it's tricky to buy fruit and time it out when they are going to be ripe enough and it doesn't help it's the start of winter now.

Even though he and his girlfriend are raw food and vegan, you can still learn from them why high carb/high calorie/low fat is where it's at and they also promote a "backup diet" of a staple of high starch foods (rice, potatoes, etc) if you can't do the mostly fruit thing.

They always STRESS getting enough calories or you will fail any diet you try.
