One other thought
I thought I would mention vitamin D. I didn't see that mentioned in your list of things you've tried. I don't think it is a cure-all, but it might be an important component.
My cousin is a nurse practitioner, and she and the doctor she works with are some of the few traditional medical professionals who are interested and informed in alternative medicine and natural healing (for example they do colon hydrotherapy in the office). They learned sometime last year at a seminar about how rampant Vitamin D deficiency is, particularly in about a 1-hour-drive area around where we live -- and we have a lot of sun here. You'd think people would have pretty decent levels, but we don't. Worse than people up north in the cold! They started measuring the blood serum Vit D levels of everyone coming in their office and were amazed at how absurdly low everyone was - only 3 people had even the "bare minimum"! And there was a correlation with people who had diabetes (and maybe certain other conditions) having the lowest levels of them all. So they have been prescribing Vit D for some of these patients, especially the diabetics, and seeing good results. It isn't that they want these people to have to TAKE Vit D, but they can't very well tell them to get sun exposure because that doesn't work around here for some reason.
Also, if memory serves me correctly, I think vitamin D is a hormone, or perhaps it directly interacts with hormones and their formation.
You might want to get your hormone levels tested (and also serum vitamin D) even if you don't want to take any sort of hormones. I know ZRT Laboratory does a saliva test, your doctor may have to order/prescribe it for you. I'm sure there are others, as well. Just getting the results would let you know where you are and what you are working with, and how best to go address your specific issues naturally!
