You are correct that most Gallstones are cholesterol stones and that most Gallstones that are detected are diagnosed using ultrasound. The key word, though, is calculi. Only cholesterol stones that are calcified are readily detected by ultrasound because the stones have actually hardened to such a density that the sound waves bounce off them and provide an image.

Intrahepatic and duct cholesterol stones (and pigment stones) that are soft, and therefore not calcified, are generally not detected by ultrasound. The following article includes more info than just the diagnostics of stones, but the answers to your questions are contained within the article.
