Yes, you do need to use a spermicide with the cap...but far less than what you use with a diaphragm...I actually found the contraceptive film to be great for this because you can fold it, insert it into the cap and no mess whatsoever! Of course, there's still the issue of chemicals though.

Unfortunately, the cap is a bit trickier to insert and remove...and you do have to be more careful that it is positioned properly since it is smaller and requires more precision.
