I can only tell you how I treat mental illnesses, including adrenal fatigue or Chronic Fatigue Syndrome.
In psychonutritional therapy in which I am trained and that I practise for many years, it is a cardinal principle that you FIRST eliminate physiological/medical causes and THEN look at psychological factors at a later stage, if at all necessary.
If a person is depressed and diabetic, you cannot help that person unless you address his diabetes FIRST. Diabetes becomes manageable (not curable) by stabilizing a person's blood sugar levels. Hypoglycemia is perhaps the most common illness affecting mood disorders, but this is not to say that hypoglycemia is the only factor. There are many others.

I am retired now and spend my time helping people overcome mood disorders with the help of educational articles I have written over the years for the benefit of my clients. People can help themselves through education, and it does not cost them one cent.
There are plenty of other approaches, but I am more familiar with the psychonutritional approach.
If people do not like my approach, they will have to look for other therapists.
