I think your problem could be your diet. Low-fat/fat-free is disastrous for the human body and my adrenal fatigue started when I was on a crazy low-fat diet.
Saturated fat and cholesterol are the precursors for every hormone the adrenal cascade makes, not to mention Vitamin D. This is why, when you don't consume cholesterol, the liver attempts to make up the difference by manufacturing it instead.
Saturated fat should comprise the biggest part of your diet and is a big part of why I feel better today than I did then. Fat is not the enemy, carbs are.
It might also be a contributing factor to your gallstone problem. Unless you eat fat your gallbladder does not release bile.
Gallstones form from bile salts that have been siting in the gallbladder that aren't being released. The best way to have a gallstone free bladder is to eat a high fat, low carb diet.
My first major panic attack, like yours, occurred when I was eating basically no fat, all vegetable and lean protein diet. It was also when I felt my worst. My mental faculties left me, I developed schizophrenic like symptoms, and severe hypoglycemia. Switching to a very high saturated fat/cholesterol, very low carb and moderate protein diet has eradicated most of my problems. I still have some anxiety/hypoglycemia because my adrenals still don't make enough cortisol, but otherwise I feel so much healthier and my body shape went from the pear to the apple shape.
I also think your particular diet is probably deficient in all of the essential fat soluble vitamins. Particularly vitamin D, which has some implications in panic.
Just some ideas, again, not meaning to attack your lifestyle or diet.
