90/45 is a MAP (Mean Average Pressure) of 60. Ideally this figure should be 93. My blood pressure at it's highest is 90/60, which is a MAP of only 70. I don't think an MAP of 60 is anything to worry about. When I fast my MAP goes lower than 60 and I have never been close to death. Also, the low figure of 45 is the blood pressure measurement of when your heart is relaxed (diastolic), not contracted (systolic).
MAP = DP + 1/3 (SP - DP)
I think your doctor may possibly be a slight alarmist. This is what happens when you deal with people who do not understand the fasting process.
Clinical significance
MAP is considered to be the perfusion pressure seen by organs in the body.
It is believed that a MAP that is greater than 60 mmHg is enough to sustain the organs of the average person.
If the MAP falls significantly below this number for an appreciable time, the end organ will not get enough blood flow, and will become ischemic.
