This probably doesn't belong in the fasting forum but, with your metabolic problems and being underweight, maybe you should give the high fat approach a try? The Homo Optimus diet claims to be beneficial for insulin-related and other auto-immune problems and says that it will make you lose weight if you are overweight, but gain weight if you are underweight.

It is based on the following principles:

- fats are the most metabolically efficient because they are easy to digest and can be used by the cells without much conversion work being needed
- high blood Sugar is not good but a certain amount of carbs is necessary for the body to function efficiently
- eating more protein than necessary burdens your body

This program recommends a daily intake of:

- 1 gram of protein per kilo of your ideal weight
- 2.5-3.5 grams of fat for each gram of protein
- 0.8 grams of carbs for each gram of protein
