My ultrasound showed very tiny stones, mostly sludge, and not much at that, yet what excruciating pain I had! The pain is caused by the body trying to pass the stones/sludge through the narrow ducts. In my case, I never did have any big stones in my gallbladder(proved by post surgery lab report), but ultrasound has been known to miss them. And since I've passed a couple thousand stones in the 8 cleanses I've done,since my GB was removed, that, IMO, proves that pain could also be caused by the liver trying to pass stones.
