I think that many of the actual trolls are ones that are seeking a reaction. There is no excuse to call others names or instigate reactions based on differing healing modalities.
For example, one user states that an acidic environment in the body causes disease and claims that increasing pH would assist in healing disease.
Then, another poster could explain that there is no research supporting the pH theory, or hypothesis, of healing.
Another poster could start naming the original poster an imbecile, idiot, or crusty old man full of 40 pounds of feces that can't get a date, then put down the increasing pH idea as dumb without any evidence. That last poster is a troll, someone that entices a reaction to satisfy a personal need to create drama without any credible evidence, especially. The trolls tend to go off topic and flame, often hiding behind numbers.
I agree, someone that has a different opinion does not warrant a troll title though.
