You mention cutting carbs, but that is likely not enough. The diet needs to be followed very strictly in order for you to be able to control your symptoms. Of course, I don't actually know what your diet is like now, so I can't say with certainty that this is the reason for your troubles.
If there is another cause of your yeast problem, then you really need to address that as well.
I would definitely recommend that you find out more about heavy metals. They may be the real root of your troubles. I think the most obvious question is whether or not you have ever had silver fillings. They're not the only source of metals (mercury in this case), but they are one of the most common.
I've had a number of the symptoms you described, and they mostly went away when I followed the diet strictly, but I didn't really feel better until I had finished a few months of chelating.
