Hit it with terbinafine cream. That stuff seems to be, from personal experience, dozens of times more potent than topical azoles. Also, a mixture of the two may be an idea to explore, since there have been studies that terbinafine+itraconazole/fluconazole systemically manage to hit azole-resistant strains hard, and much harder than systemic terbinafine alone at that!
