let me know if you want it in publisher (so you can edit symptoms to personalize) or in adobe (you can't edit it in this form). If you want it in adobe, let me know if you want the symptom list blank (and you can write your own in) or if you want the generic list included.

Please, all, keep in mind, I put this together myself, so it "ain't perfect" ;) (not bad though, if I do say so myself lol)
