Wow that's some story o_O. The last part when you say you're trying to be more aware of your drinking and your social group is a good step forward. Choose your friends wisely and make sure you're with a group of friends when you go out clubbing. Overtime, you may begin to regain your confidence. Remember, just take it slow if you have anxiety attacks. Progress is progress, and I know you can overcome this. Hopefully I can come up with more advice later but right now I'm still shocked.
