You did in the 19th century and early part of the 20th, but not any more. Today's education system ensures that anyone with the required mental capacity can become a heart specialist, orthopaedic surgeon, acupuncturist, or whatever, regardles of how poor a home they came from.

That apart, dont you think there is a considerable difference between the right to drink water and the right to practice medicine on others?
