First of all, don't eat or drink anything but water. If you have fear of fasting, don't fast. It is important that you trust your body. So basically now you are back to day one.

You do not have pain around your heart, but seems to me in your stomach or transverse colon.

Just rest, drink warm water and stay warm, all you need to while water fasting.
