If your tooth is in that bad a shape then it might not be salvageable. Bacteria contribute to decay due to lactic acid production. You have tried all the nutrients which assist repair but without success. Have you tried adjusting your oral bacteria terrain? It might turn the tide by negating the harmful affects of the normal oral bacteria which might give your teeth the opportunity to heal by circumventing the effects of decay producing lactic acid.

ProBiora3 oral probiotics which are produced by many manufacturers contain 3 bacteria which do not produce lactic acid. They also produce hydrogen peroxide which can help destroy competing bacteria.

I got some made by garden of life which are mints. The amount of bacteria is fairly low so I made a yoghurt out of them which produced a decent set yoghurt. Note the due to the low amount of bacteria it takes a long time to produce the first yoghurt but subsequent batches using a previous batch complete considerably quicker which indicates the substantially higher quantities of bacteria. You could then work this around your tooth so that they can destroy any harmful bacteria. The yoghurt might also supply useful nutrients.

The bacteria strains are:
Streptococcus rattus JH145™, Streptococcus oralis KJ3™ and Streptococcus uberis KJ2™
