The "best" cod liver oils recommended by weston price I dont believe fall into the "fortified" group. Additionally the FDA doesnt regulate supplements very closely. A visit to the consumerlabs.com will show you quite frequently the label claims on supplements are not accurate.
That being said all supplements are not created equal and handling and packaging methods can have a huge impact on product viability.
As for vit A, yes I agree beta carotene is a precursor and for many its all thats needed. Even in its precursor form (which IS limited in its conversion to pure vit A as I stated above) is still a powerful antioxident. Its conversion can be affected by many factors so its important you try what works for you. That being said Vitamin A in itself is not a single entity but a reference to a group, some of which are predominantly found in veggies and another predominantly found in proteins like egg yolks/liver. Anyway you slice it there is no magic vitamin A (or D for that matter) that is going to be the panacea for everyone. I know some people who get no benefit at all from synthetic D3 but 30 minutes under a sun lamp regularly and they have great 25(OH)D levels.
I think the biggest takeaway is there is no one size fits all recipe and no nutrient works in a vacuum, rarely can a single element be added with little or no concern for its nutritional counterparts.
