This is actually a fairly common complaint and it seems that the more carbs people eliminate from their diet, the worse the intolerance becomes. I also had this a few years back when I was trying to follow very restrictive diets.
parasites and candida are both very likely suspects but probably long before these became a problem, your entire digestive system was developing problems over years and years. It is quite possible that your high protein, high fat diet is doing more damage to your digestive system and the longer you stay on it, the more damage you will do. There is a very small percentage of people who can thrive on this type of diet but they will feel very good and healthy when eating this way and they will not have carbohydrate intolerance, they just feel good eating this way. Most likely your body very much needs the very things you are avoiding, I know you feel terrible eating them, but you probably have to force yourself to at least add in very small amounts of easy to digest carbohydrates if you ever want to make any progress. All varieties of squash are very soothing to the digestive system, try adding small amounts of this to your diet, even if it gives you a reaction, I would continue with eating at least some every day, eventually the reactions will ease up slightly.
