What I hear is that some people need large amounts, but before going with crazy doses of ACE, I'd add some of the other things he suggests that we consider:

Pregnenolone--the mother adrenal hormone. Hg affects production of it. It makes great sense to me to include this when Hg is involved. Be careful because with Hg you don't know how the body is going to convert it. Some can end up with too much testosterone if you over do it. You can see more here.

I get followup tests from my doctor. Some use saliva adrenal tests from canary club or elsewhere.
Warnings here but consider that MDs are usually very cautious. Cutler's dose recommendations were too much for me as I started to heal with chelation. I had to back way down on my dose but I really like it, esp with a little Li.

A close friend whose hair test matched a cutler rule and some others in the yahoo chelation groups use some of it along with ACE and sometimes along with HC meds.

Licorice. I think it helps cortisol to hang around longer.

Forskolin. Not too many of the folks at Yahoo like it but some have said positive things. I have some around for very busy days when I need an extra boost.
