Thank goodness that we have Dr Fuhrman MD in this day and age.
Before perusing his current books of "Eat To Live" and "Eat For Health", I had already read and purchased his book, "Fasting and Eating For Health"------A Medical Doctors Program For Conquering Disease back in 2001, although the date of publication reads as 1995.
I knew then that this latter book is about two of the principles of Natural Hygiene-------diet and therapeutic fasting--------and where I then discovered from further research, that Fuhrman began his career in health/medicine as an "Internee" of Dr Sheltons: Fuhrmans foot was injured in his professional ice-skating career, and it was only thru NH that he was able to effect a "cure", and which then provided the motivating force in his career-change into Medicine.
He also had the insight to realize that to bring this knowledge to a wider audience he would need to qualify as a Doctor within Mainstream Medicine, as opposed to any alternative qualification, as this would carry much more weight (pun not intended) in convincing his health-peers as to the efficacy of Natural Hygiene.
He has also been able to expand on Sheltons knowledge via medical techniques and been able to "marry" the two disciplines, to the point where his medically qualified peers now totally respect and accept his methods of disease-prevention and recovery.
Hats off to him.

could be answered this way..........
True hunger as Dr Fuhrman describes, occurs when the body is free from its metabolic/environmental toxic overload...........AND where the body requires little or no energy for healing: this is achieved via water-fasting as the most efficient method, OR via a plant-based diet as promoted by Dr Fuhrman, which Dr
Shelton and his contemporaries described as an "Elimination Diet".............the diet of course takes longer for the body to achieve this goal, but it will get there eventually all the same.
Once the body is free of its toxemic condition, the body will function as Nature intended and "free-up" the energy that would otherwise be used in dealing with eliminating toxemia, plus the energy that would be needed to accomplish any healing into normal bodily tasks, and that means you will experience the true hunger he refers to.
Q. How often does this true hunger reveal itself?
A. In a good state of health as described above.......the answer is: every single day, and for most, around lunchtime AND Dinnertime, but this will also depend on the activity of the body in calorific-expenditure.
Physical exercise will "burn" more calories, and where the body will signal sometime soon thereafter, that these need replacing, resulting in an increase of the intensity of true hunger: this is why people with a sedentary occupation/lifestyle will have a diminished experience of true hunger compared to someone with a much more physically active way of life. Bragg stated that "you have to earn your food" via exercise and activity, and how right he was in saying it.
The human body is designed for activity, and where the current saying of "use it or lose it" is so apt today as it was of yesterday.
