Make sure you do prostate exam. It may not be the most comfortable exam, but it doesn't last long, and it may show if you have real reason for concern. Also, some semen will stay behind after ejaculation and when you urinate what was left behind will come out - that's probably what you see.
Other things you're mentioning don't make much sense, so maybe you're exaggerating, or maybe something's really wrong... The pain you feel could even be just psychosomatic - meaning, you feel what you fear, not the other way.
Even if there IS something wrong, it is almost certainly due to your emotions. Try to learn to control your emotions, make sure no emotion goes unnoticed, make sure your thoughts are positive, make sure you love yourself rather then criticize yourself... This may take years to really get there, and is best done with help of a good psychologist. Do NOT take any drugs, ever. You may on the other hand, try to take some vitamin C and complex B, perhaps even E. Those are harmless, but very important for all of us.
And one more thing: I don't suffer from premature ejaculation, but if I don't have sex for a few days, and if my better half is really hot and turning heads on the street, and I can't wait to get her in bed, believe me, I will not last very long. But the beauty of it is that you do it once, take a little break and then you can last a lot longer second time!
Complaining about premature ejaculation is like complaining that your first cookie didn't last very long. Well, if you were really hungry, I am sure it couldn't last very long, and that's why there are many more cookies to enjoy!
So, take it easy, learn to enjoy life, and your "failure" to last when you are trying so hard to be the sex machine, is something that deserves nothing more then - "Wow, you really turn me on..." and then chill, chat with her what is it that she likes, maybe give her some other type of pleasure and then when you're ready again, it will be the ride of her life!
