If there is a bacteria in your blood now, that is causing these symptoms, it could also affect your heart muscle.

Please go to the doctor and ask for a course of Bactrim.

I am unsure what you mean by "die-off". Do you mean that you will get die-off from taking Antibiotics . That wont really happen. You are dealing with bacteria, not parasites.

Die-off of bacteria does not produce a Herx reaction usually to my knowledge, I could be wrong though.

Probably would be an idea to find a section of Curezone that is more closely related to what you believe you are suffering, so you can get more specific advise.

Maybe do a search through the list of Curezone sections. There should be one for dental health or gum disease.
