I realize that some people may choose not to do dairy... that said,
Cultured raw dairy...
and/or
wild fermentation may be the best natural way to reestablish good gut health over time...
Organic gardening and eating of raw produce that is not "sterile"/"sanitized".
Staying away from all foods, drugs and the many other things these days that destroy the microflora... like chlorine, flouride, teflon, BPA, heavy metals, etc.
The reasons many probiotic strains that should, but do not establish residence anymore, is the damage that has been done to the intestines, specifically the mucosa and/or the structures that produce new cells for the mucosa where many strains typically attach... damage to the intestinal villi also is a factor.
As you likely know, candida can and does damage the mucosa as well... if severe, villi could be damaged as well.
it was once thought or assumed that villi damage was rare... however that thought is changing with all of the chemicals in our diets... along with OTCs and pharmaceuticals causing chemical damage to the villi as well.
A French study I ran into today found that BPA at one tenth the approved daily amount decreases nutrient absorption... due to cellular changes in the intestines.

Other microflora strains which were normally found on/in foods before commercial chemical use and sanitation practices (both of which can also damage mucosa and cellular structure) were for the most part transient... though they have found that some, like Bacillus Subtilis can establish in a healthy gut.
Bottom line - I think for the most part, in modern society, we are all damaged goods from the get go... and need to repair the damage to our bodies before we can expect to "establish" and retain a healthy gut microflora balance.
