Yes, you should always take probiotics on an empty stomach. The bacteria can not survive stomach acid (which increases when you eat.) That's why the Acidophilus Pearls are so effective. The capsules protect the bacteria from stomach acid and oxygen, and they are small enough to pass straight through into the intestines. The larger sized capsules usually aren't protected from stomach acid, and are too large too pass into the intestines (the hole between the stomach and the intestines is very small, and will only allow liquids, or small food particles to pass undigested.) So, the large capsules must dissolve in the stomach first, and become exposed to stomach acid and oxygen. That's the theory behind why the Acidophilus Pearls are so effective. No other probiotic I ever tried changed my stools from yellow/green to brown, so I question their effectiveness.
