When I have custom orders I make them myself per the request. I also have the bad habit of just putting the dosing insert into the envelope without marking it - I make my customs from the order form. If I personally emailed you with confirmation of your special request - then your order is correct. The custom formula is already written on the order by me before I send out an email. The insert is just generic.

No excuse here - but I simply forgot to add in your free Pet Wormer. I deal with 16 employees, loads of emails per day every day, all of the custom orders and so forth and so on.
