Well, if the infection is deep in the roots of the tooth, it likely needs to be somehow cleaned up. Its hard to judge the nerve damage from the x-ray so the whether or not the tooth is functional may be hard to know until it's opened up.

Choosing a root canal on it can create numerous problems in the long run(it is next to impossible to disinfect a dead tooth so infection is likely to occur and potentially proliferate to other areas of the body).

An extraction can be a better option if the tooth is dead, but also can have some consequences. You wanna make sure the extraction is done according to Dr Huggins protocol to minimize risk of cavitations.
It's also important to know the position of the roots in relation to sinus cavity - a hole between mouth and sinus can be created after extraction if roots are embedded in the sinus cavity (xray can show this)
After extraction a bridge is a possibility to prevent any shifting - permanent bridge may require the 2 neigbhoring teeth to be smoothed down to make room for it. (In some cases bridges may not be feasible).

Another possibility may be using ozone to knock the infection out. This is a new procedure in USA and there are only a couple of places testing this out.
It basically involves injecting ozone directly in the area to destroy the bacteria.
This may require several visits to keep infection at bay but can potentially save the tooth depending on its condition.
I've heard that magnetic pulsing and sound treatments (Rife) can do amazing things too.

A good biological dentist should also have a EAV device available to test the tooth for type of infection, best materials, degree of damage, and best route forward.


It would also be wise to consult a tooth chart to determine organs located in same energy meridian to find potential symptom connections.
