You need to find a good doctor. And that can be very hard to do.
Ususally clinics are very good and cheap or free. You may just need a small cut on the bottom where there's a little line of skin attached too tight and that may be what's causing all your problems. If the foreskin doesn't go back it can't be cleaned properly.
Don't be shy. I know it's hard, but think about what women have to go through!
