I reread your post and you mentioned that you stopped using flouride. You could use Burt's Bee toothpaste as it contains a calcium/benonite clay base.

You also stated that the molars continue to get cavities. If a high decay rate is an issue, it is caused by streptoccacus mutans that are a bacteria that makes plaque 'sticky' and destructive to tooth structure. A good way to reduce this type of bacteria is to use xylitol products as this natural 'sugar' reduces this bacteria. Spry toothpaste and xylitol containing gum (Trident) is available. Xylitol has been shown to reduce decay by up to 60%.

I still believe your sensitivity is bite related and a simple occlusal adjustment may be all that is required. The dentist can check this by placing a piece of carbon paper between your teeth while you bite/chew on it and file the filling or crown down wherever it hits prematurely. Your teeth should only touch during chewing and swallowing. If you notice your teeth are touching or clenched during the day other than when chewing/swallowing, you may need a bite guard.
