I have known numerous people with similar conditions and they healed using one of two general approaches: allopathic or alternative. Though, you can combine them.
Based on the limited information and photos provided, this appears to be a possible mite issue (very common today) with concomitant yeast, fungal or bacterial infection.
Several species of mites can cause this condition and they typically bring along a yeast or fungal infection.
If possible, I would try the allopathic approach first since it could quickly resolve condition.
Other questions needing answers regarding your history:
When did this start? How long have you had this condition?
Do you have an idea as to what caused it?
What were the circumstances in your life when it started?
Have you had this before? If so, how did it resolve?
Do you know anybody else with similar symptoms? Friends, family, coworkers, ...
What products do you use for your face, beard, mouth/teeth?
Do you have other symptoms involving the esophagus or mouth?
Do you have any itching or other issues on your face and/or scalp, especially involving the areas with facial hair?
Did this start after you grew a mustache and beard?
Did you notice any insect bites prior to or during infection?
Do you have any symptoms on your hands, fingers or arms? Itching, pustules, dry skin, etc...
Do you have any other health issues?
ALLOPATHIC TREATMENT - 5 prong treatment:
1. IVERMECTIN: (0.2 - 0.3 mcg/kg, oral, once a day, 2-5 days)
Optional: can also include a topical 1% Ivermectin cream, apply 3-4X/day.
Can have this cream compounded (1% Ivermectin in VERSA BASE. Can include 5% - 10% DMSO). Or, use 15% Thiabendazole cream, can include DMSO.
2. ANTIBIOTIC:
Bactrim DS (1 tablet, X2/day) or Doxycycline (100 mg, x2/day), 5 days
3. PREDNISONE: (40 mg/day, 5 days) anti-inflammatory, anti-allergy
4. FLUCONAZOLE (150 mg/day, 2 days) anti-fungal, anti-yeast
5. POSSIBLE ALLERGIC REACTION: allergic reaction can be caused by mites, facial/hair/mouth products or, to food/drink you are currently using.
* Eliminate current facial, beard/mustache, mouth products.
* Eliminate the foods/drinks that you know to cause obvious adverse reaction.
* Can also try an
Elimination Diet for 3 weeks or eliminate common food/drink allergens (nightshades, nuts, grains, dairy, fruit juices, etc...).
OTHER:
* Facial hair will definitely exacerbate and can cause reinfection.
Remove mustache/beard to eliminate probable source of pathogen(s) until infection completely clears.
And, regularly follow strict facial and oral/teeth hygiene protocols:
FACE:
Use an anti-inflammatory, non-chemical facial wash or soap, X2/day. Pat dry.
Apply anti-inflammatory, antimicrobial facial/neck/lip cream - Emuaid Max or Keys MetaCare soap with the Keys RediSpray lotion (professional brands).
Use these products if you are not applying the topical Ivermectin cream which you would apply to the lips and entire face and neck, 2-3X/day - always at bedtime.
Wash hands well before washing face or applying creams.
Frequently wash hands throughout the day and do not touch the face unless applying creams, medications or washing. Use a tissue to touch face and lips.
MOUTH: do not use chemicals; use natural products; include peppermint, cloves and oregano; can include other antimicrobials; brush teeth, gums, tongue, all tissues and swish - Uncle Harry's products cover all bases (mouthwash swish/gargle and/or oil swish, baking soda toothpaste with essential oils, essential oils for mouth/teeth, etc...).
* Include high quality probiotics after meals and/or at bedtime 4+ hours apart from
Antibiotics . And/or, eat real fermented organic sauerkraut after every meal.
* Avoid animal contact until infection clears.
* Avoid areas you think could be contributing to problem (e.g., farms, gardening, dirt, stables, homes with animals, toxic environments like painting rooms, clothing, bedding, linens, etc...).
* Wash all linen, clothes, bedding, etc... on sanitary cycle to destroy any possible mites or other pathogens.
* Avoid topical and internal chemicals.
* Eat a very simple and clean diet high in vegetables.
* When in public avoid touching face and lips and wash hands frequently.
* Do not use any other lip balms or products.
* Do not kiss anybody until condition resolves.
* Can use gloves to apply topical medications or creams until issue resolves.
* Sanitize living environment.
The above protocol may need to be followed for a longer period (1-3 weeks) and/or repeated as many times as necessary to resolve infection (usually 3 cycles).
A longer treatment period is usually needed if infection has been persistent for a long time or is resistant.
FUTURE: other possible causes to address if treatments do not work - other
parasites (e.g., worms, myiasis); fungal/yeast infection; allergic reaction (often caused by parasites, foods, chemicals or other toxins); and, a mouth/tooth infection which can involve all sorts of pathogens including the ones mentioned.
NOTE: some resistant mite infections can take up to 6 months to resolve in which case the allopathic or alternative treatments would be modified.
Follow-up visits are needed to assess condition.
NOTE: I would try to find an alternative health provider who specializes in
parasite and/or fungal skin treatments. Will probably need to see several before finding someone who will actually address the cause. At the very least, they could identify a fungal component (it looks like it). E.g., experienced Chinese/Herbal, Ayurvedic, Naturopath, or Holistic MD. A Tropical Dermatologist - specialist.
They may want to include a prolonged systemic anti
parasite and/or fungal program along with a topical. You want someone who knows what they are doing and will take a scraping to look at it under a microscope - this is standard practice in Dermatology.

ALTERNATIVE DIY APPROACH
1. To make it simple, first try the lotion noted below, since that cleared several people of their condition.
For most, it took 2-6 weeks with consistent application to completely heal.
A. BROAD SPECTRUM LOTION + DIET + SUPPLEMENTS:
They consistently applied a broad-spectrum anti fungal /anti
parasite / anti mite lotion on the lips, 3-5 times/day or more. Consistency is key.
* LOTION: (NAHS makes a good strong solution of this called FUNG-E-CLENZ): extra virgin organic olive oil, Wild Oregano Oil P73, Wild Sage Oil, Wild Cumin Oil, Wild Myrtle Oil. (Later, if needed, you can add peppermint, eucalyptus, or Lavender essential oils to this mix. There are others).
* ALKALIZE: must alkalize body if acidic = urine pH 7 - 7.3.
* DIET: absolutely no sugar. Avoid high meat, coffee, cheese, vegetable oil intake. Increase vegetables, especially leafy dark greens (make sure produce is properly washed to eliminate any parasites, eggs, cysts, bacteria, etc... on the produce).
Eat lots of anti-fungal foods, herbs and spices.
* IMMUNE SYSTEM - strengthen:
Take a good Ayurvedic immune booster from India.
Take high quality probiotics - either take them at bed time (best time) or, after meals. Can not take too many probiotics. And/or, eat real fermented organic Sauerkraut (very high in acidophilus and other immune boosters).
* MINERALS - make sure you are taking bioavailable minerals (e.g., kelp,
Sea Salt , iodine, etc... many good brands)
* ANTI PARASITE MEDS - TOPICAL
Can also apply antiparasitic medications to lips.
Can alternate with the lotion (noted above) making sure they are applied at least 1 hour apart from each other:
Thiabendazole 15% cream (anti parasite and anti fungal)
Ivermectin 1% - 1.5% cream (anti parasite, anti larval)
Albendazole lotion (anti parasite)
Fenbendazole lotion (anti parasite)
If possible, mix DMSO with these, not more than 5-10%. This will help the medications penetrate deeper into the tissues. Apply twice since the medication has been diluted with the DMSO.
NOTE: when applying any medicated creams or essential oils, it is critical that you rub them in very, very well. Wash and dry the area first before applying.
And, must apply minimum X3-4/day.
ADDITIONAL BOOSTERS:
1. BORAX
Drink Borax water, 4 days a week
(1 Liter distilled or filtered water + 1/8-1/4-1/2 tsp Borax + capful of 3% food grade hydrogen peroxide) - drink throughout the day.
Can slowly build up borax amount.
This will address many fungal and some parasite/bug issues.
Almost everybody is deficient in Boron.
NOTE: can also try applying a homemade borax/baking soda lotion to the lips and face (Dissolve Borax and Baking Soda into a 0.5% - 1% hydrogen peroxide solution. Can add aloe vera, sulfur, DMSO, pine oil, etc.... maintain the
H2O2 (Hydrogen-Peroxid) concentration).
2. CHRYSANTHEMUM PEPPERMINT TEA
Drink Chrysanthemum Tea
This addresses some parasite/bug issues and is a natural repellent to many of them.
Can add licorice root and honeysuckle flowers (Starwest has this in a blend, called Longevity Tea).
3. BAKING SODA
Drink Baking Soda Water x2/day, morning, evening
1/2 - 1 cup water + 1/2 - 1 tsp baking soda. Can add juice from 1/2 lemon.
Addresses many issues (pH, fungal, mites, pathogens, etc..).
Can add ACV 1 tsp (addresses mites, ,other bugs, alkalizes, supplies enzymes and minerals).
4. PINE
* Some people have found good success using 100% pine gum
Turpentine or pine tar or pine resin salves. Use the real products since many are adulterated or synthetic.
These preparations are anti fungal, anti parasite, anti mite, anti bacterial.
Some pine salves also contain sulfur. Some have all three: pine oil (turpentine), pine tar, sulfur, aloe vera, vasoline.
E.g., NuStock = pine oil, sulfur, mineral oil.
* Can also take pine oil internally. Look up protocols for this (e.g., real 100% pine gum turpentine, pine essential oil, etc...). Or, drink pine essential oil tea or pine needle / bark tea.
OTHER TOPICALS TO TRY (if needed):
* Diatomaceous Earth mixed with
Bentonite clay - make a paste with water and vinegar (ACV or white distilled). Options: can add aloe vera (Aloe Life), sulfur powder,
Sea Salt ,
Epsom Salts ,....
* Other anti parasite/fungal lotions or essential oils: frankincense, myrrh, lemongrass, bay leaf, thyme,
Wormwood , eucalyptus, peppermint, orange oil, neroli, rosewood, etc....
E.g.: roller bottle with frankincense essential oil, myrrh essential oil, DMSO and Black Cumin Seed oil. Apply to fingers first then, rub on lips - do not want to contaminate roller bottle.
* Rapha Remedy (Oregano or Frankincense) - can work wonders for certain conditions.

NOTE: When trying any of these treatments give yourself at least 3 weeks of consistent application to see any results. If there is improvement, simply continue until the condition is resolved. Sometimes, the symptoms worsen initially, before improving, because you are drawing out an infection(s).
NOTE: give feedback since your response or lack of response to any of these treatments can help in identifying the cause and fine tuning future treatment.
