Try pushing yourself to do things such as work out and every day things. Lyme probably created a conscious thought of Depression and then stress piled on made it a way of life - habit.

You are still young enough to where you push yourself to doing 30 minutes of cardio a day, after a month of continuous pushing of yourself, possibly everything else will fall in to place.

It is impossible to know if the problems you have are solely due to adrenal fatigue.

The symptoms are many.

I've heard Ashwaghanda is helpful.

Start re-training your brain to follow your orders, not your brain telling you what to do. The body will follow. If your brain fights back that it is tired, but you actually are able to due it, then the body is fine. Neurotransmitters are still in a stress mode from earlier. Hard work will pay off. Your sleep will get better and your dreams will begin opening up communication ways between both parts of the brain.

The first step is the hardest but hang in there. At age 37, you should begin to enjoy life. Don't allow the past and Lyme and anything else bring you down. Time is running out, you're right. But for you, so don't let some disease get the best of you. Tell it to go **** itself and take your life back. It's a gift and if you still wake up every morning, there is still hope :)
