It sounds to me like you have strongyloides--not pinworms. Your case sounds like a hyper-infection and disseminated one. If it is strongyloides, a sense of urgency is called for.
I don't expect any type of doctor would be much (if any) help since we don't have clinical parisitologists in the U.S. It would be great if you could manage to have an Elisa strongyloides (blood test). But I would begin treatment immediately.
