What is this? (Picture) roundworms and what else?

Hello all. I am new to parasites and have been treated for what's thought to be roundworms on and off for weeks with no luck. My primary care doctor wont even test me. After heavy dosing with pyrantel, vermox and albenzadole for the day prior, this is what I end up with in the commode today.
I don't even know what to make of this. Obviously the white spots seem to be some type of round worms but what are those things in the middle? I have never seen them before. Only after this heavy medicine dose.

Sometimes when I see the white worm heads or whatever they are, there will be an orange blob of jelly in the commode stuck to fecal matter where the worms are. My diet the last few days has been strictly lipton chicken noodle soup with a hard cooked egg, water, grilled cheese, pretzle crackers and a bagels with butter.
I first had symptoms after digging my garden early spring. I live on an old farm with lots of berry buahes and catfish pond It started with stomach cramps and mild diahrrea that cleared up and then after eating mexican chipolte they started coming out when i went to the bathroom and now i feel them playing about in my colon.
Am I dealing with more than one infection? I have never in 66 years seen what i am sharing with you today. I've done plenty of juice cleanses and protein shakes in my day and have lived in remote south korea, where their food went right through me and never saw this.
Please help an old lady out. My primary care doctor won't even look at it.
