I should have been more specific about what I meant by symptoms. This is not an exhaustive list, but what I mean is things like this...
appetite - increase or decrease
sleep patterns - too much or too little, trouble falling or staying asleep,
very early waking
sexuality - desire, ability to function, appropriateness, etc,
moods - depressed, anxious, manic, mood swings, flat, etc.
hallucinations - seeing, hearing, smelling things that others do not
delusions - paranoia, grandiosity, persecution
ability to care for self - basic hygiene
drug/alcohol use - past or present
stress level - past or recent traumas, major life changes
age appropriate level of functioning - school, work, stability, etc.
cognitive function - memory (short and long term), rational, logical
thought, judgement, clear speech,
concentration, reasoning, etc.
interactions with others - relationship stability, isolation, combative,
respectful of boundaries, etc.
miscellaneous - unusual habits, ritualistic behavior, obsessions, suicidal
behavior, self-mutilation, motivation, etc.
The degree and duration is also important, as is the functioning during chiildhood...anything peculiar? difficulty interacting with others? etc.
