The symptoms you present are not easy to deal with, and will require time to achieve full health.

So lets just assess your situation.......

Adrenal insufficiency leading to Cortisone dependency. Are you still dependent?
Systemically toxic.
CFS.
Gut is a mess.
Stuck Liver with no bile.
Impaired digestion.
Parasites.
Zero energy.
Underweight.

First point, your "hypoglycemia" that occurs after 12 to 16 hours of water-fasting is not actually that at all, but the symptoms of what is known as "toxic hunger" masquerading as hypoglycemia. Low blood Sugar (hypoglycemia) occurs with everyone on a water-only-fast and is a condition brought about by ketosis. This is normal.
The weakness and shakiness are caused by systemic toxemia and the bodys attempts to throw them off: these are normal symptoms on a water-only-fast if you are toxic, but do not occur while fasting if toxemia is low or non-existent.
The feeling of "blacking out" will be due to a lowered blood pressure.

To help you further I will need to know how underweight you are: your height, bone structure and actual weight. This is vital before any further advice can be given.
