Fluctuating TSH level. Fluke or possible cause for concern?

Hi, I'm currently dealing with adrenal issues although i have a lot of hypothyroid symptoms as well. I was wondering if anyone has any ideas as to if it's possible for me to have hypothyroidism.

I've never seen an endocrinologist because my labs always came back normal even though they are the most basic ones. Last year, I was tested and my TSH was 2.380, Free T4 was 1.23, and Total T3 was 105 which were all in range. This March, I was tested and my TSH was 2.5 and my free T4 was 1.3 and were all in range as well.

While i was in the hospital in March, they drew my blood and it showed that i had a TSH of 6.32 in a range of .27-4.2. It was elevated so they retook my blood a day later and my TSH was 2.63 in a range of .27-4.2, and my T4 was 6.4 in a range of 4.6-12.

My question is, is that one instance of a raised TSH level a cause for concern or can it just be a fluke? I feel like, all the tests for my thyroid have been very basic and might not be revealing the truth. A lot of the symptoms are related to adrenal issues tohugh, so Im not quite sure.

I do experience a LOT of hypoT symptoms though, and my mom is hypoT as well. Any tips or ideas? Thanks
