Hi!

Your boyfriend had quite a bit of experience with water fasts. Every time the fast would be easier, provided healthy lifestyle and nourishment in between. On your question "how long before panicking"?
1. I read that until your fat reserves are not totally depleted - you can calculate this here:
http://www.freedieting.com/tools/body_fat_calculator.htm

- you would not be starving. You should stop if you find that all your fat stores are depleted.
2. As far as you can go through your day, allocating time for rest, or being 100% in bed, according to your individual schedule and body energy, you should not be worried unless point 1. is no more valid.
Some argue that even people without any stored fat can fast, if serious conditions are to be improved; but we want to be on the safe side, and all exception confirm the rule.
If you have that heaviness before the fast, to start with, and given that Hep C is serious condition, it is reasonable to expect it will take either numerous short fasts, or few long ones to clear it.
In any case I wish you luck, we are proud of your decision to fast!
