Water:
--how much water do you drink daily? (what kind of water? tap; filtered -what kind of filter?-; reverse osmosis (RO); distilled; spring/mineral; other)
I drink lots of bottled water, at work and home, brands I guess are mostly Ozarka and Nestle
Other beverages (daily, occasionally):
Fruit juice, peppermint tea, some black coffee. I used to drink a lot of coffee, but a few years ago started drinking peppermint tea to take it's place. Haven't switched over 100% yet. Other than water, this is really all I drink. I gave up sodas and milk a couple of years ago. I used to drink beer and wine, but gave those up due to migraines.
Diet (in any given week) - please be as specific as possible:
--white sugar? white flour? white salt?
I avoid
White Sugar . The only dessert I've allowed myself in recent years is an occassional piece of apple or pumpkin pie. I usually just stick to fruit.
I have tried to eat whole wheat breads, but don't eat much now due to a candida infection (outward signs are gone...not sure if I still have it internally).
I don't use
table salt , but do use Hain
Sea Salt (not sure how much better it is)
--processed foods?
Some hot dogs, pizzas, pasta, and frozen pot pies until recently.
--gmo frankenfoods (non-organic: soy/derivatives, corn/derivatives, canola/cottonseed oil, beet sugar)
Not that I can think of.

Either cooked oats or fruit for breakfast, daily salads for lunch, and steamed veggies for dinner (occassionally a salad for dinner).

Recently eliminated...but did use butter a lot, and ate quite a few eggs.
--meat (what kind? how much?)
** Not as of a few days ago. ** I did eat various forms of beef, with only occassional sausage. Mostly for dinner. Here I am, having gone through this MRSA episode, and a few days ago I see an AP article stating that half of meat sampled had staph bacteria in it. I became a vegetarian at that moment.
--oils & fats (what kind? how much?)
My wife cooks with olive oil. I'm not sure what else.
--what percentage of the above is organic?
Probably none.
Do you use a microwave oven? If so, how often?
The only thing I use it for now is to heat up a cup of water for tea.
I did use it for various things like pot pies and warming up chili, pizza, spaghetti sauce, etc, but I have now quit.
How "un-wired" are you at home and at work?
Cell Phone ? WiFi? Cordless phone (what mHz or gHz? DECT?); other wireless toys & gadgets?
I work with computers for a living, so I'm around wireless networks (I'm not a network tech, though).
At home, I have a wireless router (2.4 GHz) and one wireless computer hooked up about a year ago. My other computer is wired.
My family, all four of us, uses wireless phones. We no longer have a land line. It was a gradual thing...first my wife and I, then my son, and just recently my daughter.
Are you exposed to chemicals in the home or workplace frequently? If so, what?

Allergies (food, chemical, drugs, other):
Use to have very bad pollen allergies, but those aren't nearly so bad now.
I was allergic to sulfur-based drugs, and may still be...not sure. I still say I am to medical folks.
Food "reactions":
None that I'm aware of.
Pharmaceutical drugs (current/past):
Recent ones are in my original post: clindamycin and zyvox...plus an antifungal: terbinafine (used with the first clindamycin treatment).
I'm not on anything currently.
In the past, scattered
Antibiotics here and there.
Was on allergy meds for a while, Allegra D and Zyrtec. Blood pressure went through the roof...figured out it was the "D" part of Allegra. Switched to Zyrtec for a while, which made me too drowsy. I quit all allergy meds after that and just endured it.
Over The Counter drugs (current/past):
Before the prescription allergy meds, I took OTC pseudoephrine, sometimes along with aspirin or ibuprofen.
I did that for years. That was before I knew how people developed allergies.
Drugs "other" (current/past):
When I was young, I drank a lot of beer, smoked marijuana until I joined the military.
Took speed a few times. I guess that's what you're after here. That was a long time ago.
Supplements (please be as specific as possible as include 'name brands' and amounts taken):
probiotic: Suzanne's Suprema Dophilus
vitamin/mineral: cannot think of the name of it..Suzanne's brand vitamin/mineral/whole food (contains some algae, seaweed, alfalfa, etc)
Most other things I've done or tried were in my original post. The
Colon Cleanse was the P&B shake (psyllium and bentonite)
Also in the past, I drank a Chinese herb tea to get my blood pressure down...it has been mildly high for a while.

It does seem to help some.

What, if anything, are you are doing or taking now that is yielding positive results?
I had not been taking the blood pressure tea in a while, but have some, so I drank a couple of cups the other day.
Since it was supposed to be good the kidneys, I thought it might help. It did seem to help some.
I don't want to drink a lot of it until someone who knows more than me says it is OK.
What, if anything, have you done in the past that yielded negative results?
Nothing I can think of.

Nothing recent. I received a lot when I joined the U.S. military years ago, as I was deemed "worldwide deployable."
How many bowel movements daily/weekly? (color, consistency, etc)
Usually one a day, occasionally two.
Do you have a juicer? (if so what kind? centrifugal; single auger; masticating; if you don't know, what's the brand/model?)
I now have a Champion juicer. I wanted a good one...I hate cheap junk that doesn't last.
I really wanted a Green Star (so I could juice the green, leafy stuff), but that was too much money.
I figured
juicing everything else was better than nothing.
Current Health Issues (the more details, the better!):
None that I haven't explained really.
If you've consulted any type of practitioner, what did they determine or suggest? Do you agree?
I'm going in to the Doc's tomorrow to have blood taken for lab work.
Have you ever done any "cleansing"? (colon, liver, kidney, etc); if so, when and what type/product & results?
Colon cleanse mentioned above, and a
parasite cleanse (used a
Black-Walnut tincture for that).
Results: Not sure really.
parasite cleanse was just in case.
Colon Cleanse seemed to help a little.
What other types of allopathic, alternative or natural therapies or protocols have you tried (if any)? results?
I think I've already listed them.

Nothing I can think of. Never had any surgeries, just a couple of fractured bones.

Diet has only been changed for the better. Work is very stressful.

Well, I regret taking an
Antibiotic that was apparently very hard on my kidneys, which I guess were weaker than I thought.
I just hope I can recover enough for them to function correctly, and get rid of the pain.
