Was the B12 high in range or high out of range ? If it was out of range it could suggest one the following

-Chronic renal failure
-Heart Failure
-Diabetes
-leukocytosis, myelo-and lymphoproliferative disorders
-Autoimmune diseases
-Polycthemia vera
-Liver disease/drug-induced cholestasis
-Protein malnutrition
-Vitamin B12 therapy

Vitamin B12 out of range on the high side is very uncommon
