I did email my doctor's office and they returned to me with:

"eliminate/modify Sugar as much as possible"

...so apparently this is not an issue of being 100% (which would mean ONLY proteins and fats as you said). "As much as possible" sounds reasonable. If I eat the non-starchy/lowest-sugar veggies and otherwise eat meats and fats, the treatment should hold.
