With rx hydrocortisone and isocort ramping up slowly is recommended. Remember, isocort should be used only for low cortisol. To determine that, a 24hr cortisol saliva test is really the only way to tell. Once you know which periods are low cortisol, you would dose with isocort during the low periods.

The easiest way is when you find yourself forgetting a dose. When that happens, you just drop that dose. Also track your temps every 3 hrs after awakening for a total of three times. Average those three temps for your average daily temp. Track temps for at least 5 days. If your temps remain stable and don't vary for more than .3 degree, then you're ready to start weaning.

I think this is only a concern if you are on too much. A physiologic replacement dose to support the adrenals aren't a problem according to Val on the yahoo group:
