I would try to see as many infectious disease specialists I can and have them test for parasites, bacteria and viruses.

I too have crawling feelings in my scalp, in my case its toxocara worm, which is difficult to treat yourself.

Your lung problem may be associated with ascaris, but it dosent sound like typical Ascaris symptoms, moreover the chest pain is also atypical. parasites usually do not intefer much with the body but just lie and suck you dry, your symptoms seem much more intense, so it may be a virus or bacteria.
