It has really good info on hypothyroidism. Does the child get outside in the sun regularly? His vit. D levels should be checked. If they're low, there are vitamin d drops and sublingual tab that can be used for a 4 yr-old. Also some good old fashioned cod liver oil that is flavored.
You said he tested negative for diabetes, what about hypoglycemia? Is there any attempt to restrict wheat? What about healthy complex carbs like brown rice? Is that in the diet? Healthy snacks like fruit, veggies and natural peanut butter would be a good idea to keep his blood
Sugar stable. Apples and peanut butter are good too as is cheese. Has his mom tried putting the family on the Atkin's diet? Not necessarily the very strict phase one diet, but the one that restricts all simple carbohydrates like breads and some grains (white rice, white bread, cakes, cookies, ice cream, soft drinks, juices) and is heavy on protein and veggies and light on complex carbs like grains and beans.
