Are you quite sure about E. coli being the causal factor? Diarrhoea can have so many different causes, most of which are not of a bacterial nature. Anyway, if E. coli would get to your urethra, my guess is that you would sooner develop a bladder infection (cystitis) than diarrhoea; because the urethra and the bladder lining have little or no defence against bacteria, whereas the same amount of germs when ingested with urine or otherwise, would most likely be annihilated by stomach acidity.
