You did great! Congratulations. It sounds like you have a really good motivation to be healthy.
One thing I might mention is that you seem to be very food-focused. In other words, you seem to spend a lot of time thinking about and planning for meals. We all do this when we fast, both because we HAVE to have the stuff ready and made every day, but I am curious if you do this all the time? I don't have a family except for 2 cats, so it is easy for me to just eat food without much thought. Of course, I have been a vegetarian for 18 years, so eating healthy foods is kind of a no-brainer. I get hungry, and the first thing on my mind is usually a salad or stir fried veggies and tofu. I only eat junk and poison myself on purpose for special occasions (like any given Sunday...or Happy Hour Tuesdays :-P). That said, I could probably stand to plan ahead and increase my fresh veggie intake and develop better routines throughout my life.
By contrast, you have all these menus and lists of foods in your post. So its the 4th of July. I wonder if you would consider not planning for any food at all? Perhaps just chowing the best of whatever comes your way, in small amounts? Or listening to your body that day and deciding what is best for you then?
I'm not trying to say how you think is wrong, I am just noting the vast difference in lifestyle and wondering about what changes we can make to be our best.
No matter what, you rock for your accomplishment regardless of weight loss or how much comes back. You have demonstrated an astounding commitment that very few people have done.
